All form types, Case Type (which includes Case Forms, Portal Forms, Attachment Forms, and Action Forms), Contact, and Application forms, take you to this last screen, where you are prompted to create either a fixed width or a responsive form:

Fixed Width Form: Provides elements and options that use pixel values to determine sizing and positioning. The form will only be developed for a single class of devices, typically a desktop or laptop computer
Responsive Form: Provides a more fluid set of options and elements that allow the development of forms that adapt the sizing and positioning to multiple device types (Desktop, Tablet, and Mobile).
After the designer loads, you can go to the design panel to start the form design. All the features and tools are described in later sections of this chapter.
